The purpose of the school health program is to create an environment of wellness that supports student learning. Please help me by filling out a health treatment plan if your child has a chronic health condition or requires special medical treatments during the school day.

Below are forms you can download if needed if your child has specific health needs that are necessary during school hours. Students are prohibited from carrying any medications without proper parental and physician written consent on file at school (updated annually), and medications are preferably to be kept in the School Clinic. Students may not bring any medication to or from school at any time.
